About This Provider
Trailhead Community is an integrated housing community designed for adults with developmental disabilities and active seniors who want to live independently while being part of a connected, supportive neighborhood. Residents live in their own homes and share access to welcoming common spaces that encourage everyday interaction, friendships, and mutual support.
Trailhead is intentionally not a care facility. We provide housing and community, while residents choose and manage their own supports based on their individual needs. Many residents work with outside providers for services such as Medicaid waiver supports, employment assistance, or transportation, while others live fully independently. Our on-site staff are present around the clock to support building safety and foster community connection, not to provide in-home care.
Families typically engage with Trailhead by learning about housing availability, determining whether this type of independent living is a good fit, and exploring how personal supports can work alongside housing. Our goal is to create a real-world, multigenerational environment where people of different abilities live side by side, build natural relationships, and feel a true sense of belonging—today and as they age.
Trailhead Community offers both homeownership and rental options. Condominium units are available for purchase, while a limited number of apartments are offered as income-restricted rentals through the Colorado Housing and Finance Authority (CHFA). Rental units typically require income eligibility and a waitlist. Families, individuals, or referral partners are encouraged to reach out directly to learn more about availability, eligibility, and whether Trailhead is a good fit for an individual’s goals and support needs.
